Video consult
15-minute median. Itsekiri-language interpretation if needed. CHW stays in-room for rapport and safety.
Rx + care plan
Generics first, formulary-guided. A 90-day care plan for chronic cases. An elder patient’s hypertension plan is the template.
Follow-up
CHW call at 48 hours. Adherence check at 30 days. Outcome logged and aggregated to the Iwere Care registry.
