Every household member. Video or in-hub at Warri Central, Sapele, Effurun, or Omadino. Itsekiri-language interpretation always available.
Chronic medication coverage
BP meds, insulin, glucose strips, inhalers, basic antimalarials — dispensed at Omadino Pharmacy or delivered to the home by the CHW.
Quarterly in-clinic visits
Physical labs, ECGs, fetal checks, and anything the video consult cannot safely handle. CHW schedules and sends the reminder.
Quarterly home visit
A Warri Central Hub CHW walks (or rides) to the family compound every quarter. BP check on Papa, vaccines logged on the children, safety audit of the home.
Bereavement-fund eligibility
The covered family joins the mutual bereavement pool — eligible for the INC-USA standard congress-wide contribution if a covered member passes.
Annual impact report
One anonymized report per year with photo (consented), adherence summary, and the list of consults you funded that year.
